Why Follow-Through Matters

At a community health event, AAYHA identified a man experiencing homelessness whose blood sugar was greater than 500. He also had low blood pressure and blurred vision.

He was immediately directed to emergency care, where he was found to be severely dehydrated and in renal failure.

But the referral was only the beginning.

For more than four months, AAYHA continued helping him access healthcare coverage, medications, cataract surgery, and follow-up care.
