Understanding Health Literacy: A Key to Empowering Communities.
What is Health Literacy? According to the Centers for Disease Control and Prevention (CDC), health literacy is defined in two ways depending on the perspective. Personal health literacy is the degree to which individuals can find, understand, and use information and services to inform health-related actions. On the other hand, organizational health literacy is how organizations can enable personal health literacy for individuals. Why is it Important? According to the Center for Health Care Strategies, nearly nine out of ten adults struggle with health literacy in the United States. This creates problems and challenges for adults accessing healthcare, potentially worsening health outcomes. The impact of low Health Literacy Low health

These weren’t quick screenings; they were intentional, compassionate conversations rooted in trust, dignity, and cultural humility. In total, 14 community members received one‑on‑one assessments, and the stories they shared revealed the deep structural barriers that continue to impact health outcomes in underserved communities. Key Impact Highlights Three individuals were found to have uncontrolled diabetes One community member had an HbA1c of 10.4; another was unaware they had diabetes at all One individual reported chest pain and was referred to the Emergency Room after not seeing a clinician for more than 3 years Blood pressure cuffs and glucometers were distributed to support ongoing self‑monitoring and chronic disease management More than half of participants were referred for follow‑up care with a PCP or the Austin Family Health Center Oral Health + Medical Health: A Critical Connection Because this event was hosted at the KinderSmile Foundation Community Oral Health Center, our team also provided education on how uncontrolled medical conditions — especially diabetes — directly affect dental health. We reinforced how mouth health and body health are deeply connected, helping community members better understand how chronic conditions can influence their overall well-being. This education was a powerful bridge between KinderSmile’s mission and AAYHA’s clinical work — reinforcing that whole‑person care begins with understanding how oral health and chronic disease are intertwined. Stories That Show Why This Work Matters One woman, nearing 60, shared that she had avoided her doctor for over two years due to fear and mistrust. Despite receiving medications through mail order, she had no idea whether they were helping or harming her. Her blood sugar was elevated, and her chronic conditions were unmanaged. Our team helped rebuild trust, provided education — including how diabetes affects oral health — and committed to supporting her as she reconnected with her primary care provider. A young woman in her 20s, overwhelmed by life stressors, had turned to vaping daily. She had
Empowering Women’s Health Through Community, Compassion & Collaboration
On May 5, 2026, the Middlesex–Union Regional Chronic Disease Coalition and the Middlesex County Public Health Center brought our community together for a powerful purpose: to empower women through education, preventive screenings, and access to essential health resources. Thanks to the leadership of Ruby Garcia, Community Outreach & Education, and Twyla Paige, Health Education Coordinator with the Middlesex County Office of Health Services, this event became a true reflection of what community health can look like when partners unite with intention and heart. A Day of Impact With our community partners, the event successfully provided: 29 Breast Cancer Screenings 21 Cervical Cancer Screenings 2 Prostate Cancer Education/Screenings 14 Skin Cancer Screenings 18 HIV Screenings 14 Vision Screenings (with 5 referrals) 21 High Risk Health Assessments, Blood Pressure & Diabetes Screenings 13 STD Screenings 45 Community Members Reached Breakfast and lunch were generously provided by Hola Doctor and Catholic Charities–Unity Square, ensuring that participants felt welcomed, nourished, and supported throughout the day. AAYHA’s Role: Advocacy in Action As a participating partner, All About You Healthcare Advocates (AAYHA) brought licensed clinicians and volunteers who performed 21 high‑risk health assessments. Our team provided: Free automatic blood pressure monitors Free glucometers Hands‑on education using the teach‑back method Literature on signs and symptoms of hypertension and diabetes Guidance on when to contact a primary care provider or seek emergency care Education on the long‑term consequences of uncontrolled chronic conditions But our work didn’t end there. AAYHA made 8 referrals to Saint Peter’s University Hospital Clinic 3 referrals to the Navigator / Navigator Exchange Program – Center for Family Services for health insurance assistance These referrals ensure that care continues long after the event — because advocacy is not a moment; it’s a journey! A Moment That Changed Us One of the most powerful parts of the day came when our team encountered women who spoke Indonesian and Mandarin — languages none of our clinicians spoke. Instead of turning them away, we turned to our community partners. Within minutes, nurses who spoke their languages stepped in to help. This moment reminded us of a truth we see every day: Language and culture are not barriers — they are realities that must be honored if we want true health equity. Serving these women inspired our team deeply. It reinforced our mission to ensure that no one seeking care is ever turned away, regardless of
Advocacy in Action: Bringing High Risk Health Assessments to Our Community
On Wednesday, April 15th, 2026, All About You Healthcare Advocates (AAYHA) partnered with the Elizabeth Coalition to House the Homeless at the Elizabeth Public Library to bring essential healthcare services directly to community members who often face the greatest barriers to care. This event reflects our unwavering commitment to meeting people where they are and ensuring that every individual—regardless of circumstance—has access to the knowledge, tools, and support needed to protect their health. During the event, our team conducted 18 blood pressure and blood sugar screenings, along with comprehensive health risk assessments. When individuals presented with elevated readings, we provided electronic blood pressure monitors and glucometer devices to support ongoing monitoring and early intervention. These devices speak three different languages, helping to remove communication barriers and ensuring that community members can confidently manage their health. Our Licensed Healthcare Clinician provided one‑on‑one education to each recipient, demonstrating how to properly use their device and then having each individual repeat the steps back to confirm understanding. This “teach‑back” approach is a cornerstone of health literacy—and it ensures that every person leaves not only with a device, but with the competency, confidence, and clarity to use it safely and effectively. During screenings, two individuals were found to have dangerously high blood pressure and were immediately referred to Trinitas ER for urgent evaluation. One community member required a heart catheterization. Our clinician remained involved throughout his hospital stay, ensuring he received the care, medication, and follow‑up support needed for a safe discharge and a pathway to ongoing primary care. This is the heart of our work: standing beside individuals from the moment a risk is identified through stabilization, treatment, and connection to long‑term care. This event is a testament to the dedication of our team and the power of community partnerships. We extend our deepest gratitude to Quadeesha Harper and Mariline Jean from the Elizabeth Coalition to House the Homeless. Your commitment to serving vulnerable residents strengthens our shared mission to expand access, restore dignity, and create meaningful, lasting change. Together, we are building healthier communities—one screening, one conversation, and one empowered individual at a time.

Since 2021, we’ve walked alongside individuals and families across New Jersey—meeting them where they are, listening deeply, and advocating fiercely so they can navigate the healthcare system with confidence and dignity. Our vision is bold and clear: healthcare equity must be a right, not a privilege. 2025 by the Numbers: What We Accomplished Together Your support helped us deliver life‑changing advocacy and education to communities facing some of the highest barriers to care. 400+ families empowered through health advocacy, screenings, navigation support, and education 240+ individuals referred to clinics, insurance agencies, and essential community services 130+ medical devices distributed to support chronic disease management 95 referrals connecting clients to ongoing medical and social support 40% of patient interactions lasted 30+ minutes, reflecting deeper engagement 200+ clients identified with high blood pressure and 50+ with high blood sugar 80%+ of services supported uninsured, low‑income families, seniors, immigrants, and individuals with chronic conditions Expanded presence across six counties in New Jersey! Achieved a 34% follow‑up rate, our highest since 2022 These numbers tell a powerful story: our communities face high rates of chronic disease, low health literacy, and overwhelming barriers to care. AAYHA continues to fill the gap with culturally responsive, accessible, and compassionate support. ❤️ When Advocacy Becomes Life‑Saving Care A 60‑Year‑Old Man’s Story: From Crisis to Stability At a community health event in New Brunswick, our team screened a 60‑year‑old man experiencing homelessness. His blood sugar was dangerously high—over 500—paired with low blood pressure and blurred vision. He was immediately sent to the emergency department, where he was found to be severely dehydrated and in renal failure. Over the next four months, AAYHA helped him: obtain charity care insurance access insulin and blood pressure medications receive cataract surgery stay connected to follow‑up care Without intervention, his condition would have continued to deteriorate. Instead, he regained stability, dignity, and hope. Preventing Crisis Through Advocacy A 27‑year‑old man facing mental health challenges reached out when he was

We’re building bridges to lasting health, one step at a time. 🙏 Gratitude in Action We extend our deepest thanks to every organization, volunteer, and supporter who made this day possible.
