On June 9, 2026, the KinderSmile Foundation, Inc. welcomed All About You Healthcare Advocates, Inc (AAYHA) (https://allaboutyouadvocates.com/) as a medical partner at their Trenton Community Oral Health Center to deliver essential health screenings, education, and navigation support to community members who are too often overlooked by the healthcare system.
Under KinderSmile’s Foundation (https://www.kindersmile.org/locations/trenton-dental-home/) leadership, our licensed clinicians provided high‑risk assessments for heart disease, hypertension, diabetes, and barriers to care — spending an average of 30–45 minutes with each person. These weren’t quick screenings; they were intentional, compassionate conversations rooted in trust, dignity, and cultural humility.
In total, 14 community members received one‑on‑one assessments, and the stories they shared revealed the deep structural barriers that continue to impact health outcomes in underserved communities.
Key Impact Highlights
- Three individuals were found to have uncontrolled diabetes
- One community member had an HbA1c of 10.4; another was unaware they had diabetes at all
- One individual reported chest pain and was referred to the Emergency Room after not seeing a clinician for more than 3 years
- Blood pressure cuffs and glucometers were distributed to support ongoing self‑monitoring and chronic disease management
- More than half of participants were referred for follow‑up care with a PCP or the Austin Family Health Center
Oral Health + Medical Health: A Critical Connection
Because this event was hosted at the KinderSmile Foundation Community Oral Health Center, our team also provided education on how uncontrolled medical conditions — especially diabetes — directly affect dental health. We reinforced how mouth health and body health are deeply connected, helping community members better understand how chronic conditions can influence their overall well-being.
This education was a powerful bridge between KinderSmile’s mission and AAYHA’s clinical work — reinforcing that whole‑person care begins with understanding how oral health and chronic disease are intertwined.
Stories That Show Why This Work Matters
One woman, nearing 60, shared that she had avoided her doctor for over two years due to fear and mistrust. Despite receiving medications through mail order, she had no idea whether they were helping or harming her. Her blood sugar was elevated, and her chronic conditions were unmanaged. Our team helped rebuild trust, provided education — including how diabetes affects oral health — and committed to supporting her as she reconnected with her primary care provider.
A young woman in her 20s, overwhelmed by life stressors, had turned to vaping daily. She had never been told about the risk of complete lung failure. After a 45‑minute conversation with our clinician, she threw away her vape on the spot and asked for help reconnecting with her primary care and OB/GYN providers.
Another young woman, living with diabetes, had an HbA1c of 10.4 and blood sugars in the 300s. Through our partnership with KinderSmile’s Foundation Community Health Workers, she was connected to primary care and received a glucometer with hands‑on education.
These stories are not exceptions — they are reflections of the lived experiences of many in our community.
Our Work Continues
AAYHA health navigators will continue reaching out to every individual identified as high‑risk or referred for care, helping them move from screening to follow‑up, connection, and continued support. Our commitment does not end when the event ends — because no one should have to navigate the healthcare system alone.
