On June 27, 2026, All About You Healthcare Advocates (AAYHA) had the privilege of partnering with All Nations Church (ANCNJ) in Elizabeth, New Jersey, for a community health event focused on education, prevention, early intervention, and healthcare empowerment.
At AAYHA, we believe everyone deserves access to healthcare information, resources, and support that empower them to make informed decisions about their health. Community events like this allow us to bring healthcare advocacy directly to the people we serve by meeting individuals where they are, building trust, and promoting healthier futures.
Addressing Health Risks Through Education and Screening
During the event, AAYHA clinicians and volunteers conducted High-Risk Health Assessments for 25 community members. These assessments provided an opportunity to identify potential health concerns while educating participants about chronic conditions that disproportionately impact many communities.
Participants received education on:
- Diabetes
- Hypertension (High Blood Pressure)
- Stroke Prevention
- Heart Disease Risk Factors
- Kidney Disease Prevention
Our team discussed the importance of early detection, routine monitoring, healthy lifestyle choices, and consistent medical follow-up to reduce the risk of serious health complications.
Providing More Than Information: Tools for Better Health
Healthcare education is powerful, but education combined with action creates lasting change.
To help participants actively manage their health, AAYHA provided:
8 glucometers for individuals identified with pre-diabetes or diabetes
7 electronic blood pressure monitors for participants with elevated blood pressure readings
Each recipient received personalized education on:
- Proper device usage
- Understanding blood glucose readings
- Monitoring blood pressure trends
- Recognizing concerning health changes
- Scheduling appropriate medical follow-up
Our clinicians also focused on practical lifestyle strategies that individuals could realistically incorporate into their daily lives, including:
- Finding opportunities for exercise despite busy schedules
- Making gradual dietary changes
- Reducing risk factors associated with chronic disease
- Building sustainable healthy habits
These conversations reinforced an important message: small, consistent changes can lead to significant improvements in overall health and wellness.
Advocacy Beyond the Screening Table
At AAYHA, our commitment to the community does not end when the event concludes.
Community members were provided with guidance and referrals to help them access ongoing healthcare services, including:
- Establishing care with a primary care provider
- Obtaining health insurance coverage when needed
- Accessing preventive health services
- Understanding the importance of routine follow-up care
- Navigating healthcare resources within their community
