On Saturday, November 16, 2024, our team partnered with the New Jersey Department of Human Services’ Commission for the Blind and Visually Impaired, through their Project Better Eye-health Services and Treatment (BEST), for a health fair in New Brunswick, NJ. The event was focused on providing comprehensive diabetes screenings to individuals in the community. The day was a huge success! The health fair saw an enthusiastic turnout, with attendees from Middlesex, Somerset, and Mercer counties. Over 90% of the individuals we served were uninsured, while the remainder were underinsured. Improving Healthcare Access and Quality Via Personalized Advocacy

On a mission to improve health and well-being of those who struggle to access healthcare, All About You Healthcare Advocates provides health education, health assessments, and health navigation services to bridge the gaps. However, this tall task can only be accomplished by a passionate and knowledgeable group of experts who understand our complex healthcare system. Who Are Our Rock Stars? Our dedicated and compassionate team keeps our community members front and center. Tanesha Owens, our founder, was motivated to start the organization following her personal experience with a chronic condition and realization of the daily struggles with work and living faced by someone who is ill. Tanesha is a board-certified nurse practitioner and patient advocate. The organization’s operations and community outreach efforts are overseen by Katherine Villacis, who is passionate about working on purpose-driven initiatives in the non-profit world. Our clinical efforts are led by Ka’Necia Phelps who has more than a decade of nursing experience under her belt and is passionate about helping minority populations and community members. Our Board Member Le’Shonna Bruce has been steeped in healthcare for a long time, with experience as a pharmacy tech and now as a licensed practical nurse. Le’Shonna is responsible for the quality of care received by the pediatric patients that she oversees, and she brings this experience to the services that we offer at All About You Healthcare Advocates. Her goal in the near further is to start and oversee a pediatric advocacy program for parents with kids with special needs and illnesses. Sharon Stoeckel, a public health executive with expertise in the health care and insurance industries, runs the organization’s volunteer services group. Sharon leverages her proficiency in leading cross-functional teams and her leadership and collaborative skills to efficiently manage our organization’s volunteers. “I am committed to promoting health equity and improving access to healthcare for underserved communities. This passion drives my volunteer work with All About You Healthcare Advocates, where I focus on ensuring that vulnerable populations have the resources and support they need,” according to Sharon, who’s been with the organization since May 2024. Mission Focus Health: Navigating Patients and Families Toward Wellbeing

Lending our voice to those who are vulnerable or underserved is at the heart of our mission at All About You Healthcare Advocates, and we achieve this by providing a broad range of healthcare services to our community members. Importantly, we do not have a one-size-fits-all mantra – quite the opposite. We provide personalized assistance to our clients based on their individual needs. Case Study: Wrap-Around Advocacy Our founder Tanesha Owens, a nurse practitioner and patient advocate, has been actively helping a family navigate their healthcare needs. Mr. and Mrs. T reached out to Tanesha around the time Mrs. T was discharged from a rehabilitation facility. Tanesha helped them set up home care services including: • Nursing • Physical therapy • Occupational therapy Unfortunately, like we often witness with our health system, the initial agency failed to follow through, leaving Mrs. T without these essential services for two weeks until Tanesha intervened. Mrs. T underwent a valve repair procedure and needed assistance with selecting a suitable rehabilitation facility that met her preferences, such as a private room, quality food services, and top-notch facilities. Doing the research to identify a suitable facility is not easy, especially for an older couple. “My role is to provide the family with detailed information and ratings to help them make an informed decision,” according to Tanesha, who helped them with the process. Mr. T says he owes a lot to Tanesha. “My wife needed significant medical care and I couldn’t imagine being able to manage the logistics,” he said, from both the care and insurance point of view. The family often reaches out to Tanesha to share concerns regarding the quality-of-care Mrs. T receives. When Mrs. T was scheduled to be discharged from her second rehabilitation facility, Tanesha coordinated with the social worker to ensure a smooth discharge process. Additionally, Tanesha helped Mr. T, who is the primary caregiver, contact his insurance company to confirm benefits, such as free transportation for Mrs. T’s return home. While the rehabilitation center informed Mr. T that insurance would not cover transportation, Tanesha intervened and confirmed that it was covered. According to Mr. T, Tanesha helped them avail of services that they did not know were covered by insurance, such as transportation to, and from, medical appointments. “[Tanesha] visited us often during the hardest times and spoke on our behalf and helped us schedule appointments, figure out insurance coverage,

All About You Healthcare Advocates plays that key role by providing healthcare advocacy services and are the voice for patients and family members. What Do We Do? Unity in Union County: Elizabeth Coalition to House the Homeless “The Community Health Fair”

Friday, February 28th was a first for All About You Healthcare Advocates!! Under the leadership of Kathy Villacis, Director of Community Outreach, we partnered with the Elizabeth Coalition to House the Homeless for their community health fair hosted by the Elizabeth Public Library. The Health Fair provided health screenings, focused health education, and local referrals to resources. The Community Health Fair served 15 attendees primarily from the City of Elizabeth, NJ. This wonderfully diverse group of people represented Latino, Haitian, and American ethnicities as well as Hispanic, Black, and Caucasian racial groups. Nearly all of the individuals were uninsured without identified access to preventative healthcare services. Interestingly, this community’s commitment to improving SDOH is evident in that the clients we served had already initiated the state aid process. The work never stops there. Every individual who attended receives a follow-up contact to ensure he/she has connected to services, and to determine what further supports he/she needs. Moving into a new target area may allow us to identify and compare resource and advocacy gaps amongst our neighboring counties. We want to extend a special thank you to the Outreach Coordinator, Ms. Quadeesha Parker for coordinating this event. Also, the warm welcome from the Elizabeth Public Library that is supporting the expansion of our reach into Union County. We are excited about our new unity in Union! Key Stats: • Over 90% of participants were uninsured and underinsured, but already in the application process with NJ Family Care/Medicaid • 15 participants served: 7 Latino, 3 Black, 3 Caucasian, and 2 Haitian • Five (5) individuals who screened positive for hypertension were provided with digital sphygmomanometers and training on proper use • Health literacy was addressed with each participant focused on individualized concerns
